Job summary
We have an exciting opportunity to recruit a band 4 Cancer
Care Coordinator to join our colorectal cancer team for 12 months. This
exciting non-clinical role will be based in Durham and will underpin the
delivery of personalised care and supporting the cancer pathway for people
affected by colorectal cancer.
We are looking for a highly motivated individual who has
excellent communication skills and a passion for delivering high quality care
who can provide the nursing team with administrative support alongside
emotional support and signposting for patients.
You will be qualified to at least
NVQ Level 3 or equivalent and will have experience working independently. You
will also have good IT skills and experience working in a healthcare setting.
You will also be able to demonstrate resilience, compassion and a willingness
to embrace innovation.

Main duties of the job
You will be based with the colorectal
cancer specialist nursing team where you will assist with the day to day
running of the service. This includes:
Supporting the Multi Disciplinary
Team meetings and outcomes to ensure the timely management of patients on a
cancer pathway
Delivering personalised care
through the use of Holistic Needs Assessments, referals to support services and
close working with the Macmillan Information and Support Team
Liaising with community and
primary care teams to facilitate the timely management and treatment for
patients
Dealing with enquiries from people
dealing with cancer, these may be face to face, over the phone or by email.
Visiting hospital departments to
offer support to patients or family members as needed.
Some travel across sites is expected.
