An exciting opportunity has arisen to apply for the role of
a Band 6 Colorectal Care Clinical Nurse Specialist within County Durham &
Darlington NHS Foundation Trust.
The successful applicant will support the
Macmillan Clinical Nurse Specialist team to actively plan, implement and
evaluate expert nursing care and provide specialist advice and psychosocial
support to patients and carers with colorectal cancer across the range of care
settings within the trust. You will work in collaboration with the Colorectal
Consultant Surgeons, Oncologists and other members of the multidisciplinary
team.
Main duties of the job
The post holder will have responsibility for supporting and developing the colorectal cancer nursing
service for patients diagnosed with, and undergoing treatment for
colorectal cancer. The post holder will have the skill set
to work autonomously when required, provide support, information and advice to
patients, carers and staff on all aspects of colorectal cancer and treatments.
Working as part of the multi disciplinary team, and as a
patient advocate, we support the patients at all parts of the colorectal cancer
pathway.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
