Job summary
The post holder will be responsible for the delivery of
evidence-based care to patients with a tracheostomy/ laryngectomy within County
Durham and Darlington NHS Foundation Trust.
The post holder will be able to work as an autonomous
practitioner, providing specialist support and clinical leadership to oversee
the delivery of safe, effective, person-centered care to all patients with
tracheostomies/laryngectomy, ensuring care is provided by appropriately trained
staff.
The post holder will provide a continuing supportive service
inclusive of information and advice within hospital and community, to patients
and their carers, and to staff supporting patients with a tracheostomy. They
will act as an expert resource for the multidisciplinary team to ensure each
person receives the highest standard of person centered care
They will take the lead and develop training and educational
programs for the clinical areas where patients are cared for with a
tracheostomy. They will support staff to achieve the relevant competence to
manage this patient group in line with local and national guidance.
Main duties of the job
The successful candidate will work as an autonomous
practitioner and will work closely with our ENT/ head and neck team, to provide
specialist, expert nursing services to patients who have a tracheostomy.
Acting as an expert resource in tracheostomy care, utilizing
their skills to provide advice and education to support staff, patients and the
wider multidisciplinary team.
To be a visible point of contact to offer support to staff
overseeing care delivery to patients to ensure all needs are met for patients
with a tracheostomy.
Identifying patients in the Trust with a tracheostomy and
ensuring all staff caring for these patients have a clear understanding of
their duties to provide 24-hour safe and effective quality care to this patient
group. Establishing and building links with community teams to ensure fluid
safe transfer of care on discharge from hospital.
Taking a leading role in the development, implementation and
review of clinical policies and protocols Trust-wide regarding tracheostomy
care, which is evidence based and in line with national guidance.
