Job summary

Ward 31 at Darlington Memorial Hospital is a busy Trauma and Orthopaedic Unit caring for patients with trauma injuries and is also the designated area for patients admitted with head injuries.

The ward is a fast pace enviroment and can look after some very complex patients.

The discharge Coordinator role is to help support the nursing team in ensuring that all patient discharges and maintained to a high standard and also to assist with the complexities envolved.

Main duties of the job

The main duties of this role will include:

Supporting nurisng staff with all discharges, including very complex ones.

Liaising with all parts of the MDT such as Doctors, nurses, OT and physio.

Building good relationships and making referrals to social services and other support teams.

The role will involve a lot of autonomy and self organisation.

  • Leadership & Management modules

  • HCA experience
  • Demonstrate the ability to liase with other parts of the MDT including other care services such as social care.

  • Knowledge and undertanding of patient mobility requirements for rehab areas.

Desirable

  • HCA experience in a acute Hospital setting or in community hospitals
