Job summary

We have two exciting new roles for experienced pharmacy technicians to apply and enhance their clinical skills and knowledge. Working within either the surgical or acute medicine clinical pharmacy units, these unique and pioneering roles have a new and important responsibility in validating the prescribing of discharge medication prescribed by our clinical pharmacists. Ideally candidates will have already achieved their clinical diploma, however applications are welcomed from those with clinical experience relevant to the role as you will be supported with internal training and development and the opportunity to complete the clinical diploma whilst in post.

Based on the Medical Admissions Unit at Chorley Hospital or the Surgical Wards at Royal Preston Hospital you will be an integral member of the ward based multidisciplinary team (pharmacist prescriber, clinical pharmacist, medicines management pharmacy technician and medicines management assistant) providing patient-centred medicines optimisation services. The role also includes assisting in medication reconciliation, medicine supply (inpatient and TTO), patient counselling, assessment of patients own drugs, ward and department based dispensing and accuracy checking and line management. We have recently introduced a clinical pharmacy prioritisation whiteboard to assist our team to prioritise their patient workload and reduce risk

Working hours are between 8:00am and 5:30pm Monday to Friday on rotation with each shift being 7.5 hours and 30 minute lunch break. There is also a requirement to work one Saturday or Sunday every five weeks (you'll then get a day off the following week) and one bank holiday a year. Weekend and Bank Holiday shifts have additional pay rates. As a Trust, we support agile and flexible working and will consider requests taking into account the needs of the service. Job description

Job responsibilities

1. Provide a clinical and medicines management service to the patients, wards and departments of the Trust hospital(s) in accordance with evidence-based practice, clinical guidelines and professional, pharmacy and Trust policies, including:

  • Clinical validation of prescribing of medicines for discharge
  • Medicines Reconciliation
  • Assessing patients own medicines
  • Supply of medicines
  • Processing medication for discharge
  • Counselling/educating patients on their medication and to improve concordance
  • Supporting the referral of patients e.g. Discharge Medicines Service
  • Identification and resolution of prescribing errors/discrepancies

2.In collaboration with the Clinical Pharmacy Unit Lead Pharmacists, plan and co-ordinate the daily clinical inpatient pharmacy services using the clinical pharmacy priority whiteboard to inform plans and actions.

3. To support the governance arrangements for safe medicines management including:

  • The audit of medicines storage against National, Professional, and legal standards providing evidence and report to Trust.
  • The audit of Controlled Drugs against National, Professional, and legal standards providing evidence and report to Trust.
  • Supporting the nursing teams with actions resulting from audits
  • Supporting the investigation and review of Datix incidents relating to medicines
  • Implementing risk minimisation schemes
  • Audit of medicines processes and feedback areas of improvement
  • CD destruction and assistance with incident investigation and reporting to CDAO

4. Provide expert advice, leadership and direction, ensuring professional and service standards are met.

5. To support the delivery of medicines management training, development and competency assessment for pharmacy and nursing staff; maintaining quality and standards by contributing to the development and implementation of new services, processes and procedures to deliver services in line with Professional, Local and National directives.

6. Work in close conjunction with the Dispensary Team Leader and Pharmacy Stores and Distribution Team Leader over matters concerning pharmaceutical supply and storage of medicines.

7. To provide line management, leadership and direction to the Pharmacy Technicians and Medicines Management Assistants

8. Maintain competencies and provide a sessional, weekend and bank holiday commitment to Trust Pharmacy service.
