Applications are invited for a Consultant Physician with an interest in Frailty and Community Geriatrics at Lancashire Teaching Hospitals.

This is an exciting time to join a progressive and dynamic elderly medicine department with supportive colleagues, working together with both acute and community partners to improve the care for our diverse local population.

These 2 new posts will lead the day-to-day running of the virtual frailty ward (VFW) newly established in 2022. The post holders will also work on the acute frailty assessment unit helping to deliver daily ward rounds. An additional session of community work is included in the job plan although how this session is delivered can be very much at the discretion of the new postholder.

Main duties of the job

The virtual frailty ward (VFW) is a new service established September 2022. The postholder would be expected to lead the VFW each day through virtual ward rounds and MDTs.

The VFW will provide a safe alternative to hospital for patients living with frailty through community-based acute health and care delivery.

Virtual wards provide acute clinical care at home for a short duration (up to 14 days) as an alternative to care in hospital. Patients admitted to a virtual ward have their care reviewed daily by a consultant practitioner (including a nurse or allied health professional (AHP) consultant) or suitably trained GP, via a digital platform that allows for the remote monitoring of a patient's condition and escalation to a multidisciplinary team.

The MDT will aim to include pharmacists, GPs, nurses, NHS consultants, social care teams, mental health teams, voluntary sector organisations.

About us

The Elderly-Frailty Department provides acute elderly medicine services and a dedicated Acute Frailty Unit. Our Surgical Liaison (POPS) services provide proactive in-reach into GI surgery, Vascular and Major Trauma areas. There is an established specialist outpatient falls service. The development and expansion of the Elderly Medicine and Major Trauma services is high on the Trust's agenda.

The Acute Frailty Assessment Unit is a new unit opened August 2020 next to ED at Royal Preston Hospital. It currently has 10 beds and daily ward rounds are led by our two new frailty consultants. In addition there are 2 junior clinical fellows. Because of its close proximity to ED the consultants are able to provide ED in-reach on a regular basis. Appropriate patients can then be transferred more efficiently from ED straight to AFAU. There is a daily AFAU MDT each morning after the ward round.
