Job summary
We are looking for an enthusiastic and motivated Mental Health Link Worker to work in the PCN , (GPs practice within the Hindley area). The PCN is the first contact from the primary sector who access people to mental health services when people need it most and provide supportfor people who might not traditionally meet the threshold or criteria of existing mental health services. The team consists of a Mental health Practitioner, Assistant Psychologist and work closely with the GPs.In this role you will support individuals within a community setting to enhance their health, wellbeing, and quality of life, assisting them to connect with and access mental health services, voluntary sector services and community-based activities and support. The role willbe delivered through working as an integral part of both primary and secondary care services, to connect individuals to appropriate support at the earliest opportunity. To provide in-depth knowledge to colleagues in the wider system about specialist mental health services as wellas community-based activities and support. To help individuals navigate a range of mental health services and community activities/services in line with their individual needs and circumstances.In addition to this, the Mental Health Link Worker will provide education and training to other staff (e.g. volunteers/peer support workers/community link workers).
Main duties of the job
To contribute to the clinical discussion of people using the PCNservice through attendance at regular MDT meetings.
To identify people's goals collaboratively, and plan/implementevidence-based interventions to use strengths and overcomeobstacles to goal attainment aimed at recovering socially valuedroles and social inclusion.
To use coaching methods and motivational interviewing techniquesto discuss needs and jointly identify opportunities, appropriatesupport, and actions.
To hold a caseload of people who use the PCN service aiding them toconnect to relevant community support services.
To work alongside volunteer community connectors and peersupport programmes to provide additional support for people toaccess support and/ or activities where required.
To practice within the relevant clinical guidelines and operatingprocedures and be legally responsible and accountable for allaspects of work.
To participate directly in any rota'd activity within the team, includingassessment and follow-up clinics, and provide support and guidanceto other colleagues undertaking the role.
To maintain clear and comprehensive electronic clinical records, toensure information is accurate and complies with Data Protection Actand Trust standards. To provide written reports detailing theprogress of mental health interventions.
