Trainee Advanced Clinical Practitioner

Job summary

An exciting opportunity has arisen to join GMMH Home Based Treatment Team and Urgent Care services in Central Manch as a Trainee Advanced Practitioner.

The Advanced Clinical Practitioner will be a senior clinical member of the multi-disciplinary Home Based Treatment Team at Central Manchester providing short-term intensive community support to help stabilise individuals and prevent hospital admissions . The team engages assertively with service users to manage crises while minimising disruption to their lives. The role will also support the Urgent Care senior leadership team with developments and education across the division.

Trainee Advanced Clinical Practitioners will develop into a autonomous independent practitioner who will lead in the care delivery in advanced skills in assessment, diagnosis, formulation and treatment. In addition to these advanced clinical leadership skills there will also be an expectation that the successful candidate will lead on research, education and training across the service.

Main duties of the job

Ability to practice at an advanced clinical level and exercise high levels of judgement, discretion and decision-making in clinical care.Provide clinical leadership, expert practice, advanced knowledge, and strategic direction on all aspects of rehab mental health .Providing clinical leadership and management leading to the provision of safe, quality, effective care.Improving workforce knowledge and practice through the provision of an inpatient education and trainingLeading improvements in the quality of care and safety through service development, evaluation and audit.

Job description

Job responsibilities

The post holder will provide advanced and expert level of practice (including non-medical prescribing)within the Home Based Treatment Team (HTT) leading to improved patient outcomes.

For Trainee Advanced Nurse Practitioners, it is essential that you reach the criteria in the general Apprenticeship eligibility. The Apprentice ACP will act as a professional resource for the multi-disciplinaryteam, patients and carers within the scope of their practice and competence, The Apprentice ACP willunderstand professional accountability and legal frameworks for advanced practice and be professionallyaccountable and legally responsible for all aspects of own work. The Apprentice ACP will demonstrate safepractice in line with Trust policies and procedures.

The post holder will support the clinical and operational team to deliver on service improvements in thequality of care and safety through service development, evaluation, and audit.

They will be an integral part of the MDT supporting clinical decision making and supporting the servicepathways, developing nurse led clinics and providing brief interventions.

The Advanced Practitioner will hold a case load and will lead assessment, formulation and prescribing ofcare planning including discharge planning within agreed protocols and procedures.

The post holder may additionally lead on practice and service developments. The post holder is required todemonstrate clinical and practice expertise via interventions, supervision and modelling good practice.

To support improving workforce knowledge and practice through the contribution to a HTT education andtraining programme across Urgent Care teams.

The core hours are Monday to Friday 9-5, although some flexibility, including weekend / BH working may berequired.

Advanced Clinical Practitioner (ACP) positionsshould be accessible to bothnurses and Allied Health Professionals (AHPs) in line with the Multi-Professional framework.
