Job summary
Prescott House Community Mental Health Team in Salford is a busy, dynamic, successful and supportive integrated health and social care service. We work closely with service users, carers and families to deliver person centred, recovery focused interventions .
With direct line management, clinical supervision and daily support from the from the team, the post holder will deliver, maintain and co-ordinate timely and appropriate interventions for a caseload of service users living in the community within the framework of CPA, in line with relevant clinical pathways as outlined within the CMHT operational procedure.
The team has a clear focus on staff development, health and well being, encouraging joint working, sharing training and knowledge.

Main duties of the job
Prescott House Community Mental Health Team in Little Hulton, Salford is an integrated health and social care service and strives to provide quality recovery-based support to people with complex and or enduring mental health problems. Under the line management and clinical supervision from within the team, the post holder will maintain and co-ordinate timely and appropriate interventions for a caseload of service users living in the community within the framework of CPA, in line with relevant clinical pathways as outlined within the CMHT operational procedure.
You will be responsible for the care coordination of a caseload, assessing service users mental health and risk and managing your caseload according to the CPA policy.
This includes respecting the individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.
The role involves duty assessment and triage on a rota basis, undertaking depot clinics and home depots and some weekend and Bank Holiday working on a rota basis
