An excellent opportunity has arisen to recruit a Community Psychiatric Nurse/Care Co-ordinator intoour busy Community Mental Health Team for Later Life
The area covered by the post is North Manchester
We are looking for an experienced, committed and motivated professional to work with older people who are living with dementia or functional illnesses.
Main duties of the job
- Care Co-ordination under CPA
- Complete Memory Assessments (training will be given)
- Administering depots
- Complete initial assessments
- Participate in Duty Rota
- Carer Assessments
