Job summary

Salford Living well is a new service developed to support individuals with mental health problems. The aim is to widen access to mental health support across Salford.

We have co-produced and co- designed a service that is accessible to all. We place people's aspirations and goals at the centre of all we do. We believe in person centred care that is tailored to individual needs. We have co-designed an initial conversation and my story my plan tool to support individuals manage those bad days and to focus on their own personal and social strengths

Our main principles are trauma informed care, strengths based approaches and we are solution focused. We are a multi-agency team and value all contributions to the team. We have a peer mentor programme that is continually being developed and co-production is at the heart of Living Well.

Main duties of the job

We are looking for an innovative, adaptive and caring individual who wants to be part of a multi agency team and who will support the ongoing developments in Salford Living Well and primary care.

As a Mental Health Practitioner in the team, you will be expected to support and participate in the national evaluation program attached to Living Well.

You will be supported by a strong multi agency team, you will be part of an innovative new model of community care and you will be supported via supervision and line management and will be part of a wider collaboration.
