We are looking for an enthusiastic Mental Health Practitioner to join theMental Health Liaison Team at Manchester Foundation Trust hospital sites at Trafford and Wythenshawe hospitals.You will be expected to work on a 24/7 shift based system, covering a range of conditions including self-harm and suicide risk, dementia, delirium and other organic/functional mental health illnesses.We work closely with the Alcohol Practitioners to support and advise on dual diagnosis presentations
You will need to be a proactive and resilient practitioner, committed to working in partnership with service users, their careers and other professionals to provide a truly person-centred service. You must be able to demonstrate excellent communication skills and the ability to work flexibly to meet patient and service needs

Main duties of the job
We provide bio/psycho/social risk assessments and care planning for patients presenting with any mental health concern and/or risks. We support our medical colleagues in the management of medically-unexplained symptoms and also provide care to patients with a mental illness admitted to medical wards for physical health needs, supporting patient experience and ensuring effective, evidence-based care.
You will have demonstrable experience as a Registered Mental Health Nurse/Occupational Therapist/Social Worker with experience in adult mental health and/or later life care groups. In addition, we provide anAll-Age service to the A&E Department/Urgent Care Centre and accept referrals for children and young people. We will offer the successful candidate appropriate training and supervision to work with children and young people and also with those in later life. You will be supported by our team approach in all aspects of assessment and discharge care planning.
