Job summary

We are looking for an enthusiastic and motivated Mental Health Link Worker to work in a newcommunity mental health service in Wigan called Living Well (LW).

The Living Well Service willenable timely access to mental health services when people need it most and provide supportfor people who might not traditionally meet the threshold or criteria of existing mental healthservices. The team will be multidisciplinary and will include third sector organizations. We willalso be supporting several peer mentor roles within the team.

You will be supported by members of the leadership team including a team leader, operationalprogramme manager and clinical lead. CPD opportunities are available and personalinterests/skills will be supported to develop and grow.

The Wigan Living Well Service will be compassionately led, trauma informed, and recoveryfocused. Living Well has co-produced key values and principles that all staff and peers share.As a result, our model is humanistic and person-centred. Main duties of the job

In this role you will support individuals within a community setting to enhance their health,wellbeing, and quality of life, assisting them to connect with and access mental healthservices, voluntary sector services and community-based activities and support.

The role willbe delivered through working as an integral part of both primary and secondary care services,to connect individuals to appropriate support at the earliest opportunity. To provide in-depthknowledge to colleagues in the wider system about specialist mental health services as wellas community-based activities and support.

To help individuals navigate a range of mentalhealth services and community activities/services in line with their individual needs andcircumstances.

To undertake initial conversation / assessments of people as part of the LWduty system in accordance with agreed operational procedures. In addition to this, the MentalHealth Link Worker will provide education and training to other staff (e.g. volunteers/peersupport workers/community link workers).
