Job summary
We are looking for a social worker to start with our Living Well Team. The post holder will undertake a key worker role and assessment within the team.
The Living Well Service will enable timely access to mental health services when people need it most and provide support for people who might not traditionally meet the threshold or criteria of existing mental health services.
The team will be multidisciplinary and will also include third sector organizations. The areas that this team will cover:
First Living Well Hub - Farnworth/Kearsley, Rumworth, Bolton Central, Breightmet, Little Lever.
Second Living Well Hub - Turton, Halliwell, Horwich, Westhoughton, Blackrod & Chorley Roads.
You will need to have experience of triaging, signposting referrals, and experience of multiagency working.
Bolton Living Well Service will be compassionately led, trauma informed, and recovery focused. We will also be developing peer mentor opportunities for people who have been through the service. CPD opportunities are available and personal interests/skills will be supported to develop and grow.

Main duties of the job
