CYP Crisis Youth Support Worker

Job summary

This is an exciting opportunity to transform the way in which acute mental health care is experienced by children, young people (CYP) and those who care for them across the Wigan and Bolton area. Working as part of the CYP Crisis Team, you would work across the two care pathways; The Rapid Response Team, which is for up to 72hrs and the Home Intensive Treatment Team which is up to 6 weeks intervention.

The service operates 7 days a week 365 days a year.

Main duties of the job

As a member of the multi-disciplinary team the service aims to enhance the current community care package for young people with complex needs who are at risk of hospital admission. The team also works closely with the inpatient wards in identifying young people who are suitable for an early discharge pathway.

The post holder will develop continuing relationships with young people and families, delivering elements of their care plans. Direct intervention work with young people will take place in the home environment as well as clinical environments, including emergency care settings and community settings including schools/educational settings.
