Job summary

  1. Provide clinical leadership, supervision, support and consultation to nursing staff and MDT staff in the CMHT, in line with relevant clinical pathways as outlined within the operational procedures.
  2. Manage and evaluate a caseload in line with relevant clinical pathway (Nurse lead clinic) as outlined within the operational procedure and will include: -
  • Assessment
  • Formulation
  • Therapeutic intervention as required
  • Signposting
  • Close working with all colleagues
  • To be responsible for a complex caseload
  1. Act as a duty assessor

Deputise for the Team Manager in his/her absence and take on any delegated management duties as required.

Main duties of the job

To demonstrate high-level communication skills at all levels

To demonstrate the application of specialist skills and knowledge in order to maintain professional competence and fitness to practice as a Senior Practitioner for Nursing

To undertake and record comprehensive risk assessment, and implement evidence based therapeutic interventions based on the recovery model and personalisation agenda.

To take responsibility for managing a ring-fenced caseload of service users, with complex mental health needs.

To undertake local audits and reviews in line with national/local standards and service developments as required.

Ensure systems are in place for, Duty, triage and assessment, Allocation, transfer and discharge / closure of case responsibility

Participate in bronze on-call management rotas as required.

Act up for the Team Manager as required.

To lead on Team Management Reviews and take responsibility for implementing action recommendations arising.

To establish and maintain robust communication networks with clients, carers, team members, other health care and social care workers and external agencies.

To encourage self-help as much as possible, promoting the maintenance of skills and independence in clients.

To support health promotion and act in a consultative role/advisor to other community services/agencies and personnel in relation to individual students and their mental health issues.

To take the clinical lead for the service within your area of specialism and provide advice, guidance and support to colleagues and other professionals.

To undertake and record comprehensive risk assessments, devise and implement risk management plans for clients at risk of vulnerability, self harm or risk of physical, sexual or emotional harm to others, using the STAR V 2 Risk Assessment Tool.

To take a lead role in service developments within your area of specialism.
