Social Worker/Care Coordinator: Community MH Rehab

Job summary

The GMMH Rehabilitation Division have an exciting opportunity for a dynamic and innovative individual who are looking for a new challenge. This is a unique opportunity to be part of the newly developed Home Engagement and Rehabilitation Team (HEART).

The service aims to provide bespoke, intensive and individualised care to service users living with complex, enduring mental health problems to enable them to live fulfilling lives to their full potential.

The successful candidate will play a central role within the MDT to ensure high quality, recovery focussed care is delivered to our service users in the community.

The role involves the care coordination of a small caseload of service users (no more than 10). The post holder will lead on the assessment, collaborative care planning and provision of interventions for the individuals on their caseload. The focus of care should be around self management, optimising independence and promoting choice and inclusion as part of service user's recovery.

The service will initially pilot in Salford, with roll out across Manchester and Bolton localities thereafter. Full induction and training will be given as part of the role.

Main duties of the job

In the role of care coordinator you will have the opportunity to work intensively with a small caseload of service users who have been identified as requiring community rehabilitation under the CPA framework.

You will need to be able to establish rapport, trust and the ability to work collaboratively with service users and their carers to support their recovery and thrive in their own homes and local communities.

The role will be varied, and the ability to work creatively and flexibly will be an essential aspect of the job.You will work within a full multi-disciplinary team and participate in shared clinical decision making to determine most appropriate management plans for service users. You should be able to work independently as well, as part of a very supportive team, and have the ability to prioritise tasks. The post holder will maintain a social perspective to their work and promote social care issues within the team.

You will have the opportunity play a key role in the development of a new service, and in the organising, supervising and supporting of junior colleagues.

The service operates 7 days a week between the hours of 8am-8pm. We are happy to discuss working patterns and are able to support Mon-Friday 9-5 if that is the preference of the successful applicant.
