Job summary

This is an exciting opportunity for a Care coordinator to join our Community Mental Health Team

  • To manage a caseload of people with complex mental health and social care needs, using evidence-based practice to assess, plan, implement and evaluate interventions and document these in accordance with local and national standards and guidance.
  • To have care-coordination responsibilities for an agreed number of clients on enhanced Care Programme Approach (CPA) within the team.
  • To participate in the assessment of people referred to the team and present findings to the multi disciplinary team.
  • To provide leadership for junior staff, multi-disciplinary support staff and students through formal supervision, education and appraisal.

  • 1.Post registration qualification in a clinically orientated area 2.Psychosocial interventions qualification 3.Qualification in leadership or management

  • 1.Post registration experience. Recent experience working in either adult mental health or CAMHS.
  • Experience of and an understanding of working in a multidisciplinary, multi-agency or complex care organisation
  • Experience of working with service users with risk related behaviours
