Job summary
This is an exciting position which will focus on the physical wellbeing of our service users with mental health needs. The post holder will be responsible for the leading on the physical health care agenda within Central West Community Mental Health Services under the supervision of the Team Manager
The post holder will ensure the delivery of all physical health care CQUIN requirements and trust key performance indicators. They will act as a specialist resource and advisor to care coordinators within the community teams. The post holder will work across community settings, including clinic bases and home visits.
The post holder will be required to work, for the majority of time, as an autonomous clinical practitioner for patient groups across a range of clinical settings,. The post holder will be expected to forge strong links with the Care Coordinators, Physical Health Support staff, Primary Care services particularly GP practices.
Main duties of the job
Management of Physical health support staff.
Assessment of Physical healthcare and developing management plans
Experience of completed Physical Health investigations and associated interventions
Experience of undertaking physical health observations
Effective communication skills and experience in the management of others
Ability to act on own initiative and provide advice and support to nursing staff
Ability to liaise and communicate with a range of internal and external personnel
Ability to use clinical information/IT systems
To implement change within a clinical setting

- Participate in and provide clinical supervision sessions
- To take a lead role in the evaluation of physical healthcare guidance and to translate this into practice to ensure the service maintains excellence in practice
- To take a lead role in supporting the senior leadership team and team managers with the embedding of any new physical healthcare policy or practice

- To take the lead role in the application and evaluation of the Shared Care Protocol for prescribing anti-psychotic medication.

- Where appropriate liaise with staff at the acute hospital where patient transfer may be required ensuring all relevant clinical information is provided

- Ensure PHIT assessments and interventions are completed for service users identified in high risk groups in line with associated key performance indicators, e.g. CQUIN targets, etc
- Undertake any audits that are related to physical health care e.g. Physical health improvement tools, blood sugar monitoring equipment and resuscitation audits and ensure that any actions are completed in a timely manner
- To be competent in Immediate Life Support (ILS)
- To provide assessment and management of any chronic wounds and to make any necessary onward referrals
- To ensure that any physical healthcare risk assessments are completed in a timely manner and reviewed as appropriate
- Ensure compliance of self and others with any infection prevention and control requirements
- Liaise with medical staff as and when required
- To ensure that patients have access to health improvement interventions and to signpost as appropriate
- Participate in the functioning of the clozapine clinic on a weekly basis.
Medicine Management
- To attend the directorate medicines management meeting when required
Human resources and learning and development
Participate in the annual appraisal and PDP process and agree objectives with the Team Manager
- Attend and participate in Clinical and Management supervision as per Trust policy and Supervision framework for nursing guidance
- Provide supervision and mentorship to junior staff and students as required
- Participate in any agreed projects, service developments and investigations as agreed with the Senior Manager and Assistant Director
- Maintain professional requirements and standards for NMC and Revalidation
- Participate in any learning and development initiatives for self and others as required
