Come and work for Kingslea House Community Mental Health Team in Withington Manchester. We are looking to recruit a Band 6 Community Psychiatric Nurse to work within the Teams.
North Mersey CMHTs are fast paced, dynamic, supportive, well performing teams within the South Manchester directorate of GMMH covering a wide geographical area and varying service user and carer needs.
We are looking for an enthusiastic mental health nurse who has some experience/knowledge of working with service users in community setting, however consideration will be given to candidates who have experience and transferable skills from other areas.
This post provides a challenging opportunity to work within an established fully integrated supportive Community Mental Health Team.
We are seeking to appoint individuals who will work proactively with service users and their carers, within a multi-disciplinary setting.
We provide a really supportive working environment to assist with the transition from inpatient nursing to community.
Main duties of the job
You will hold and manage a caseload of service users and have Care Coordinating responsibilities under the Care Programme Approach Guidelines. By assessing your service users and carers needs you will be able to access and sign post to a wide range of community resources and provider services available in the Manchester area. The post holder will be expected to participate in providing quality first line psychosocial assessments as part of GMMH's Functional CMHT's duty role and work proactively in forging professional working relationships with multi-agencies. There is an expectation to work weekends /BH's as part of GMMH's Acute Care Pathway provision of care. This is on a rota basis.

- Be a full member of the multi-disciplinary team (MDT).
- Effectively manage own caseload accepting referrals from agreedagencies in accordance with agreed operational procedures.
- Undertake formal mental health nursing assessments of new clientswithin own caseload.
- Undertake assessments of clients as part of the CMHT duty system in accordance with agreed operational procedures.
- Assess, care-plan, provide interventions and evaluate and recordcommunity mental health nursing delivered to clients on own caseload.
- Act as Care-Coordinator for appropriate clients under the CareProgramme Approach (CPA).
- Provide Specialist Mental Health Nursing Assessments, Continuingcare Assessments and advise mental health nursing managementplans as requested e.g. from Local Authority Social Workers, Medical, Nursing or Social Workers from Trusts, and nurses from the PCT.
- To promote positive mental health and physical care to clients in allsituations.
- To encourage and support clients to collaborate in their treatment programme and recovery.
- To encourage self-help as much as possible, promoting the maintenance of skills and independence in clients.
- To support health promotion and act in a consultative role/advisor to other community services/agencies and personnel in relation to individual clients and their mental health issues.
- To undertake specialist mental health nursing assessments when required in line with the Mental Capacity Act 2007, The Mental Health Act and Local Authority Safeguarding Adults Procedures.
- To undertake and record comprehensive risk assessments, devise and implement risk management plans for clients at risk of vulnerability, self-harm or risk of physical, sexual or emotional harm to others, using the STAR Risk Assessment Tool.
- To personally regularly participate in Line Management and Clinical Supervision.
- To provide clinical supervision to other members of the multi-disciplinary team.
- To supervise junior members of staff in their Risk Assessment and Formulation of risk.
- Deal personally with patients, their families or carers who make a complaint.
- Demonstrate advisory skills to junior staff in dealing with complaints.
- Ensure, as far as practicable, the full involvement of service users and where appropriate, relatives and carers in the assessment of their needs and the formulation and review of their care plans.
- To act as Practice Educator for Nursing students on clinical placement within the team.
