Job summary

This is an exciting opportunity for an experienced Mental Health Nurse, Social Worker or Occupational Therapist to join our specialist service for people experiencing a first episode of psychosis.

Main duties of the job

The Assessment Practitioner will be responsible for completing initial assessments for all service users referred into the service within two weeks, in accordance within the NHS England Access and Waiting Time Standards.

The post holder will be supported with their responsibilities by the wider team including administrative support. Regular training will be provided as needed.

Job responsibilities

To complete specialist assessments for all referred service users accepted into the service, with support from the wider team as identified when required. To manage discharges from the service and transfers to other services as appropriate for service users who are not accepted for the full First Episode Pathway service following an initial assessment. To undertake risk assessment and risk management for individual service users and to provide advice to other professionals on acute and chronic aspects of risk assessment and risk management. To work as part of our integrated MDT, to attend daily meeting to feed back the outcomes of assessments undertaken, and to participate in a clinical decision making process to determine the most appropriate management plans for all service users who have received an assessment. To have knowledge of, and contribute to the team adherence to the NHS England Access and Waiting Time Standards for EIS. To have a good knowledge of the required documentation for case management, and to ensure compliance with any associated performance standards. To maintain a high standard of clinical record keeping, including electronic data entry and recording, report writing and the responsible exercise of professional self-governance in accordance with relevant professional code of practice and Trust policies and procedures. To liaise professionally and sensitively with other involved parties in order to obtain further information pertinent to the holistic assessment process. To provide advice, guidance and consultation to peer and junior professionals within the service, both individually, and by attendance at team referral and review meetings.

Qualifications

Essential

  • A qualification in Social Work, Mental Health Nursing or Occupational Therapy
  • Registration with the regulatory body as applicable; HCPC, NMC, Social Work England

Desirable

  • Evidence of robust continued professional development including training in relevant subjects

Experience

Essential

  • Demonstrable experience of working with people with psychosis & mental health difficulties such as depression and anxiety
  • Demonstrable experience of working in the NHS

Desirable

  • Experience of working within a CMHT / EIS setting and completing assessments with people who have been referred into the service
  • Experience of working with children / young people
