Job summary
We are looking for an experienced RGN with primary care experience to join the physical health team based within the Adult Forensic Service at GMMH. The successful applicant will provide high quality, holistic primary care for patients in a forensic mental health service.
The role includes delivering nursing care and carrying out interventions such as ECG, spirometry, cervical screening and immunisations; supporting our staff and service users with health education; working alongside GP colleagues and supporting and managing the physical health team.
You will report to the Advanced Clinical Practitioner who is the lead for physical health within the service.
The post holder will be based at the Edenfield Centre at Prestwich Hospital and will support the physical health lead to develop physical health provision across the service via attendance at relevant meetings, practice and procedure development and contributing to specific projects and pieces of work.
The post holder will be required to provide cover across the service as when required.

- Clinically triage GP appointment requests to determine urgency.
- Support the running of GP clinics ensuring follow up actions for the physical health team are completed.
- Manages programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed guidelines and patient group directions.
- Maintains disease registers in liaison with administrative and other department staff.
- Undertakes annual reviews of patients understanding and ability to self manage where appropriate.
- Administer immunisations and vaccinations for adults and young people in accordance with national and local programmes.
- Ensures safe storage, rotation and disposal of vaccines and drugs, and that adequate stock levels are maintained.
- Undertake procedures including: venepuncture, glucose tolerance tests, BP management, urinalysis, peak flow, spirometry, new patient health checks, pregnancy testing, stitch/clip removal, ear syringing, and wound management.
- Provide support and chaperoning to the doctors for examinations and other procedures.
- Promote health and well being, run clinics, and give general education and advice on smoking cessation, chronic disease management, and healthy lifestyle.
- Support programmes of health screening as agreed with clinical teams in relation to cardio-metabolic syndrome. Review PHIT reports from Business Intelligence and help to formulate subsequent action plans.
- Assist with the planning and implementation and teaching for staff including medical students, nurse students, nurses and health care assistants
- Act as mentor for more junior staff and students, assessing competence against set standards
- Disseminate learning and information to other team members in order to share good practice and inform others about current and future developments (e.g. courses and conferences)
