Job summary

An exciting opportunity has arisen for an Advanced Practitioner, to join the Central Manchester Community Mental Health Services).

The Central CMHT's offer a comprehensive range of mental health specialties within one multi-disciplinary team to meet the mental health and social care needs of the population of Central Manchester who require the input of secondary care mental health services.

This is an exciting time to join a dynamic and ever-developing service to drive forward recovery led care.

Main duties of the job

The successful candidate will join the service as a member of the Central Manchester Community Mental Health Servies.

The Advanced Practitioner will have a personal case load and will be responsible for the assessment, formulation and prescribing of care planning including discharge planning within agreed protocols, for service users presenting with complex mental and physical health needs. The post holder will be recognised as an expert practitioner in an identified field of practice and will advise the multidisciplinary team on expert, safe and effective models of care and treatment.

The post holder will effectively be required to cover all functions of the community division determined by the service need including, but not limited to, work in the CMHTS in the Central footprint.
