An opportunity has arisen for a number of Band 6 Registered Mental Health Nurses, Social Workers or Occupational Therapists to join us in supporting individuals in acute mental health crisis at home, as well as supporting their loved ones during this time. We would also consider supporting preceptees at a Band 5 level to meet a number of competencies and achieve a substansive Band 6 role over a 12 month period.
Staff wellbeing and development is paramount - full supernumery induction is provided for the first 4 weeks in post. You would be supported with your professional development as part of your appraisal , and regular 1:1 supervision is offered as standard.
Main duties of the job
- To provide comprehensive assessment of a service users mental state and social needs which would aid the completion of a collaborative care plan for community management.
- To provide comprehensive risk assessment, formulation and management as part of the service user's journey.
- To act as keyworker for a small caseload of service users, with support from the multi-disciplinary team.
- Cover the duty / shift coordination role on a regular basis.
- Liaison with other agencies involved in a service users care.
- Lead on the discharge or transfer of care for our service users to appropriate agencies.
- To support and supervise junior members of staff.
- The team operates a 24/7 service, therefore there will be a requirement to cover shift work - flexible working can and will be supported however possible.

The post holder will work as part of an integrated multi-disciplinary community service, with responsibility for assessing health and social care needs and providing a gate keeping role within the Salford directorate. They will provide clinical interventions focusing on relapse prevention, early discharge from hospital and improved quality of life for service users who are experiencing mental ill health and are over the age of 16. The service operates 24 hours a day, seven days a week.
Within a team approach, the post holder will key work a defined caseload, providing a wide variety of supporting treatments and interventions while service users are under the care of the home-based treatment service. The post holder will be expected to offer highly developed risk assessments and management plans with an emphasis on recovery.
The post holder will also be required to act as a duty officer on a rotational basis, taking responsibility for the triage and assessment of referrals, and the allocation of the teams work. The post holder will play a key role in the development of the service, by assisting the leadership team in day-to-day operations, organising, leading, teaching, supervising and supporting junior colleagues.
