Job summary

This is an exciting opportunity for a Mental Health Nurse, Social Worker or Occupational Therapist to join our specialist service for people experiencing a first episode of psychosis as a Care Co-ordinator.

The post holder will be required to work the core hours of 9am - 5pm Monday to Friday. There is an expectation that the successful applicant will undertake weekend working on a rota basis as required to maintain the out of hours service. (One weekend in every 3 month period) Weekends are paid at an enhanced rate.

Main duties of the job

Under line management and clinical supervision from within EIT, the post holder will maintain and co-ordinate timely and appropriate interventions for a caseload of service users living in the Salford area. The post holder will work within the framework of CPA, and in line with relevant clinical pathways as outlined within the EIT Operational Procedure.

Working in a supportive and friendly team, you will care co-ordinate a caseload of service users who have been identified as experiencing a first episode of psychosis. You will have access to daily MDT meetings to support you, and you will receive regular training in all aspects of your job role.

To complete specialist assessments for referred service users accepted into the service when required. To maintain a high standard of clinical record keeping, including electronic data entry and recording, report writing and the responsible exercise of professional self-governance in accordance with relevant professional code of practice and Trust policies and procedures. To have knowledge of, and contribute to the team adherence to the NHS England Access and Waiting Time Standards for EIP. To undertake risk assessment and risk management for individual service users and to provide advice to other professionals on acute and chronic aspects of risk assessment and risk management. To liaise professionally and sensitively with other involved parties in order to provide seamless and consistent high quality care for service users under care of the team. This includes, but is not limited to; carers, families, internal and external colleagues, and voluntary services as appropriate. To hold and manage a caseload of service users who have been identified as experiencing their first episode of psychosis, including needs assessments, care planning and delivering psychosocial interventions. To have a good knowledge of the required documentation for case management, and to ensure compliance with any associated performance standards. To undertake and record comprehensive risk assessment, devise and implement risk assessment plans for service users at risk of vulnerability, self-harm, self-neglect, or risk of physical, sexual, emotional harm to others, using the STAR risk assessment tool. To be aware of and adhere to responsibilities for people under CPA, including offering/arranging carers support. To be aware of and adhere to the NICE guidelines for first episode psychosis.
