Job Summary/PurposeThis post is offered as a permanent opportunity for an experienced band 6 mental health practitioner to work within the Health and Justice Community Services. The role will be based at April House in Manchester and will involve the case management of service users in the community with mental health problems or learning disabilities who find themselves in contact with the criminal justice system.
Main duties of the job
Work closely with a high demand cohort of service users with mental health problems or learning disabilities.Provide high quality case management to service users with complex needs.Receive referrals and actively engage person/s in a dedicated time frame.Be able work in an assertive outreach based model.To participate in the development of service policy and procedures as well as ensuring that all policies and procedures are implemented and adhered to.Support the evaluation of practice and other aspects of service provision and implementing appropriate measures for improvement.

Directly engage with individuals who are presenting with mental health problems and support the development of a multi-agency plan of care and/or intervention.Provide brokerage with local service providers to enable a joined up response which addresses underlying issues relating to mental health or other social problems.To attend meetings and participate in service development as appropriate to role.Participate in team and individual and managerial supervision.Training and ResearchMaintain appropriate data of activity and keep accurate, up to date clinical records.Liaise with other agencies and organisations across health and social care in Manchester on issues related to the service.Network with other similar schemes on both local and national levels.Participate in research activity as and when required and actively promote the concept and practical application of evidence based practice.
