Job summary
o To provide nursing and therapeutic interventions, involved in the assessment and coordination of care needs, planning and implementation of evidence based care to children, young people and their families accessing the Solar Crisis and Home Treatment Team.
o To ensure children, young people and their families have a good experience of accessing Solar.
o Solar is a partnership between Birmingham and Solihull Mental Health NHS Foundation Trust, Barnardo's and Autism West Midlands and provides Emotional Wellbeing and Mental Health Services to Children, Young People and Families in Solihull (0-19).
o Solar was set up as a service not about thresholds or tiers but about timely access to appropriate support in line with children and young people's needs. Solar aims to create a comprehensive system, designed around the needs of children and young people, which keeps children and young people healthy as well as treating those that are ill and prioritises resilience, partnership and co-production.
Main duties of the job
o The post holder will provide high quality assessment for patients referred into Solar, providing a role in risk assessment and management, formulation of need, information gathering, and offering advice and guidance to children and young people directly and those who support them (parents/carers, family and professional support networks e.g school nurses, teachers, social workers).
o The post holder will manage a defined caseload and be a care co-ordinator within the service.
o The post holder will work with partner agencies (statutory and third sector) to ensure a co-ordinated approach is had to manage the care of children and young people in Solihull.
o The post holder will ensure the most relevant care pathway and care plan is devised and where needed provide seamless transition into other Solar teams and practitioners .
