Job summary

An exciting opportunity has arisen for Band 7 Clinical Leads to join our Neighbourhood Mental Health Team working across South Birmingham working across our Primary Care Networks (PCNs).

We are looking for enthusiastic and dynamic individuals who are able to work within a multi- disciplinary team, working alongside other professionals and striving for the best outcomes for individuals within the local community that the team serves.

Our community mental health services are well-established teams, as part of the BSOL community transformation programme, we are looking to increase our neighbourhood mental teams with these roles, focussing on a presence alongside GP colleagues within PCNs. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care. The development of our Neighbourhood mental health team is an essential service which we are in the process of evolving in line with the NHS Long Term Plan.

Main duties of the job

You will be responsible for providing mental health triage, assessment and outcomes based Intervention for service users within the Neighbourhood mental health team.

You will be responsible for identifying need and best course of advice, support or intervention.

You will be responsible for promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.

Crucial to this role will be your ability to ensure robust communication across FTB/BSMHFT and primary care in accordance with each individual service users needs by provide support and advice to all colleagues with the PCN as required including participation within the local neighbourhood MDT's and those working in the voluntary sector.
