Job summary

This is an exciting opportunity for an experienced HCA who can add their physical health skills to compliment and support the wider older adult community mental health team here covering the Solihull area . You will be supported to provide a unique service within the clinic environment and within service user's homes.

This post holder completes physical health tests and monitoring for commencement of medications inc anti dementia medications and provide nursing support to the MDT. Completes home visits.

Lifestyle advice

Annual physical health monitoring

The post holder will report to care coordinator/lead clinician and GP any physical healthconcerns that may require further investigation or intervention

Post holder will contribute to meeting the trusts annual physical health target and any other targets set by the physical health or infection control committees.

Job description

Job responsibilities

To assist in service users to encourage recovery and wellbeing in both, hospital and community settings.

To assess service users for basic health parameters core physiological measures- i.e.. heart rate, blood pressure and respiratory rate where appropriate.

To plan, organise and lead groups independently and unsupervised in the community setting.

e.g. smoking /exercise groups / healthy lifestyle groups

To contribute to the clients physical health care plan, keeping all records up to date and validated by a registered staff member

To monitor the service users attending groups and report problems with individuals to the physiotherapy staff, GP medical or senior nursing staff for evaluation if they are not comfortable advising the client appropriately.

To participate in the screening of patients using screening tools developed by supervising clinical staff and clinical working groups.

To support service users to make healthy lifestyle choices about exercise and nutrition.

To signpost service users appropriately to community services where needed.

To be part of any necessary planning or reviewing of treatment progress.

To use ECG equipment and provide ECG trace readings for review by the medical staff.

To undertake venepuncture on request or for screening purposes within the health and wellbeing clinic setting.

Blood Glucose monitoring.

To refer clients onto other agencies as appropriate.

To attend relevant training, meetings, and reviews as required.

To promote, improve and provide service delivery using innovation

To be aware of and practice universal precautions when dealing with bodily fluids such as faeces, blood, or urine.

Administrative and inter-professional working

Joint working with other professionals.

To document attendance and progress of the clients contemporaneously.

To record accurate and precise documentation following any patient intervention, care planning or liaison in line with operating standards for wellbeing services.

To maintain a safe working and treating environment.

To maintain patient confidentiality, privacy, and dignity in their contact with groups of individuals and at all other times.
