Job summary

An exciting opportunity has arisen for a Band 6 CPN looking for a new challenge to work within East Assertive Outreach Team.

The Assertive Outreach Service is a multidisciplinary team that adopts a person-centred approach, focusing on individual recovery. We work with adults with complex, severe, and enduring mental health needs in the community. We work with 25 years plus and support people who have historically avoided contact with mental health services and do this using a flexible and creative approach to engagement.

This position would be suitable for a Registered Mental Health Nurse with a qualification at degree level or equivalent and registration with a relevant professional body i.e NMC with several years' community experience working with service users with complex long term needs.

The Assertive Outreach service operates over 7 days per week, 365 days per year. Our shift patterns consist of day shifts (9:00am - 5:00pm), late shifts (12:00pm - 8:00pm) and weekends (9:00am - 5:00pm). However, we can offer flexible working arrangements during these hours.

In return we can offer a friendly and supportive multi disciplinary environment in which to develop through training opportunities, regular individual and group supervision and a supportive team ethos.

The ability and willingness to travel across the Birmingham & Solihull area for work purposes is required for this role

Main duties of the job

  • You will act as Key worker to a prescribed caseload, effectively managing your workload utilising Dialog plus care planning tool in recovery focused interventions, medical reviews, engaging and developing a therapeutic relationship with the caseload.
  • You will be expected to work as an independent practitioner but also collaboratively as part of the Team. You should demonstrate the ability of working with challenging and complex issues and be able to joint work with other professionals as required to support and assist the service user & families with on-going difficulties
  • You will need to demonstrate a range of skills including effective communication, empathy, self-awareness, clinical skills, and understanding of the Mental Health Act, compassion and critical self-reflection.
  • To attend daily handovers and maintain high levels of communication within the team and contribute to the risk management processes within the service.
  • The applicant should possess a thorough understanding of the role of a CPN and the role of key worker and be able to clearly articulate this in practice, demonstrating high levels of skill in assessment, intervention and planning within a client centred recovery approach.
