Job summary
We are seeking to recruit a psychological therapist to join our Primary Care Team delivering care to the Homeless community. We are based in a GP practice dedicated to the Homeless and have well established links to all other agencies involved in the care of the homeless.
We have two Community Psychiatric Nurses in post, and we wish to develop our mental health services to provide psychological therapies to our service users, professional advice and support for our other team members and to develop integrated care pathways for service users in cooperation with the currently employed nurses, the Community Mental Health Team for the Homeless and the Mental Health Outreach Team for Rough Sleepers.
There is a focus on reducing the impact of psychological distress and risk behaviours, supporting users of the service to access other appropriate multi-agency services and offering individualised psychological treatments.

Main duties of the job
The post holder will provide engagement, assessment, triage, enhanced support and high intensity psychological interventions within the homeless primary care setting, which are equitable, non-stigmatising, accessible, and responsive to the needs of homeless adults.
The post holder will work as part of a multi-professional team and provide psychological assessments and interventions to service users from the community mental health team for homeless adults in discussion with the clinical lead and consultant.
The post holder will provide psychological assessment and treatment for patients registered at the Health Exchange and who are also engaged with CMHTs, FTB and other organisations.
The post holder will maintain and improve knowledge and skills based on best practice, particularly specialist knowledge and skills in the delivery of psychological therapies within primary care in particular to individuals with complex trauma/ personality disorder, dual diagnosis and high-risk behaviours who are difficult to engage and struggle to access services.

Clinical provision includes assessment, formulation and focussed evidence-based interventions within both homeless services as and when appropriate.

Assumes accountability for own professional actions working independently holding a case load and alongside CPNs managing mental health drop in clinic and referrals from primary care staff.
Undertaking highly specialist psychological assessments and individual/group based psychological therapies.
Provides teaching/training of psychological principles of working within team and Trust.
Provides supervision to team in trauma-informed care of complex cases. Undertakes CPD and appraisal with line manager and in supervision.
Offers peer supervision within psychological modalities and engages in own supervision and supervision groups.
Provides supervision and liaison-consultation services to the primary care team.
Undertakes governance, audit and clinical administrative activities, collecting, collating and analysing data, writing and presenting reports and undertaking project development and implementation where appropriate. Uses IT as required including statistical, database, spreadsheet and presentational.
Innovates and develops activities in response to patient need and service development.
Works independently and as part of a team within framework of trust/team/service principles, professional codes of practice etc.
Applies advances specialist knowledge commensurate with qualifications, accreditation and further training as agreed.
