Job summary

The National Learning Disability & Autism Programme has resulted in changes in the way care is managed and funded for people with a learning disability and / or autism.

We are recruiting for a committed Clinical Discharge Hub Coordinator with experience of working within the field of Mental Health, Learning Disability or Autism to work primarily within the admission avoidance /timely discharge process across BSOL.

Working collaboratively with citywide partners the post holder will ensure that citizens are supported to be safely discharged in the community wherever possible,and in a timely way. Where admission to specialist hospital is required the post holder will ensure this is outcome focussed and appropriate, that that alternatives have been considered, and that discharges are proactively planned and timely.

Main duties of the job

The post holder will be an experienced clinician or practitioner working within the mental health, learning disability and autism team (MHLDA) and will sit within the Birmingham and Solihull MH&LDA Provider Collaborative..

The post holder will be expected to work alongside senior clinical leads and colleagues to provide oversight and co-ordination of the inpatient cohort including admission avoidance strategies and facilitating the discharging of clients

The post holder will require specialist clinical knowledge of the health needs of people with learning disabilities, autism and/ or mental health as well as local and national priorities across health services and the impact of these on people with learning disabilities and autism with or without mental health needs.

An understanding of health and social care systems within the Birmingham and Solihull locality would also be beneficial. The successful candidates will also have highly developed reporting and analysis skills and be confident with having outcome focussed and collaborative conversations with a variety of stakeholders to promote patient outcomes and system challenge, working with both NHS and non NHS providers and Local Authorities.

Care reviews are being undertaken both virtually and in person, so an ability to travel is essential.

  • Registered Nurse with the Nursing and Midwifery Council (NMC)
  • Post Graduate Qualification at Master's level or equivalent.
  • Evidence of completion of INSPIRE Leadership Programme or other Leadership Qualification/Programme.
  • Qualification or demonstrable equivalent experience in Quality Improvement methodologies (QI)

Experience

Essential

  • Extensive experience at a senior clinical or strategic level in a healthcare setting or social care setting.
  • Experience of leading successful organisational change and development.
  • Demonstrable experience of leading improvement activities involving multiple clinical disciplines.

Skills/Knowledge

  • Highly developed understanding of place-based care and population health principles.
  • Very highly developed knowledge of nursing and national drivers supporting the clinical division.
