Job summary

To work with the D & F Early Intervention Manager to embed the principles of the Birmingham Older Peoples Programme.

To be responsible in partnership with key clinicians/teams for the timely throughput of BSMHFT patients in line with best practice guidance.

To address, and where necessary, escalate / resolve discharge delay.

To coordinate, advise and instigate completion of appropriate legislation documentation required for non-acute placements.

To facilitate the discharging of clients from BSMHFT using the Care Programme Approach/Care Management process in liaison with Home Treatment, Community Mental Health Teams, Community Enablement & Recovery Team (CERT), Care Home Liaison (CHL) and any other service.

To take a leadership role and proactively manage the overall resource management for Discharge Co-ordinators provision.

To represent Dementia & Frailty Services in selected clinical forums as required to support service development and delivery.

Main duties of the job

To be responsible for the timely throughput of Dementia & Frailty inpatients in line with best practice guidance.

Report and escalate any known likely delays to discharge to CERT Team manager and Clinical Nurse Manager.

To manage, coordinate and minimise the usage of any external provision.

To contribute to the leadership, service redesign, change and operational management of specified emergency care services.

To lead and promote the integration and development of services, taking responsibilities for defined areas of work.

To promote a common vision and ownership of the Trusts values and strategic objectives, advising on the implications of strategic and operational policy decisions.

To work with clinical staff in developing appropriate changes in clinical practice within the Dementia & Frailty services to support development.

To ensure that all staff in the Discharge team understand the strategic direction of the Trust and are able to contribute to its development.

The CERT team manager will agree personal objectives with the Capacity Utilisation Manager on an annual basis. This will form part of the annual review of his/her performance in the role.

To reflect on and interpret information, facts or situations which require analysis, option, appraisal and solution.

To take the role of lead, chair, organiser, coordinator or facilitator at meetings relevant to the role when appropriate.

To ensure that activity against the agreed performance management indicators for the service area is measured and communicated to service commissioners and other interested parties.

To have specialist knowledge of the local health economy and the impact of national strategies on service development.

To be responsible for ensuring that effective communication systems are in place for all staff working within the service.

To further develop own managerial skills; to maintain own continuing professional development (CPD), by keeping abreast of any new trends and developments and incorporate them as necessary into work plans.

To monitor the bed waiting list for Dementia & Frailty on a daily basis.

To be a point of contact with the wider Trust Bad Management Team.
