Albion Mill is an Intermediate Care facility based in Blackburn with Darwen. Our patients present with a combination of physical, psychological and cognitive impairments requiring the support of a dedicated MDT committed to supporting them to achieve their optimum level of independence.
We are based within a new facility which also offers Extra Care Accommodation and a vibrant Community Hub with a full range of services right in the heart of Ewood, Blackburn.
Using your excellent communication and interpersonal skills, you will develop strong working relationships with a range of health, care and community partners and expertly attend regular multi-disciplinary team meetings to ensure patients achieve their best positive outcomes.

Applicants are required to have a relevant professional qualification and current registration, excellent leadership skills and experience of working with adults including undertaking case management, care co-ordination and multi-disciplinary team working.
Main duties of the job
You are required to have a proven ability to be a team leader, and effective communication and leadership skills are essential. Your role will involve leading a team of nursing staff and working with the multi-disciplinary team to organise and deliver complex Rehabilitation programmes and ensuring the dovetailed transition of patients to an appropriate discharge destination liaising with partners as required.
You will be responsible for the management of the unit including the deployment and supervision of staff and the teaching of students.
You must have evidence of continued professional development, and up to date knowledge of current trends within Rehabilitation.
You should be confident and able to communicate at all levels.

Have continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and rehabilitation for a cohort of patients with complex neuro-rehabilitation needs.
To lead a nursing team and ensure that high standards of care and rehabilitation are maintained by the whole team
Responsibility for the setting of high standards of care and the management of a specialist Neuro-rehabilitation unit, including the deployment and supervision of staff and where the teaching of student and/or extensive nursing intervention is required.
To effectively communicate with all the Wards and Departments, within ELHT and externally, and General Practitioners to ensure timely and appropriate admissions and smooth, safe transfers and discharges.
Participate in the Band 7 out of hours shift system to provide adequate nurse management cover on the hospital site.
Effectively manage resources and maintain a budget
Effectively lead the team and ensure policies and procedures are followed
