Job summary

An exciting opportunity has arisen for a community rehabilitation band 6 physiotherapist to join a supportive and enthusiastic community therapy service as a Physiotherapist, working across community delivering rehabilitation in patients own homes following discharge from an acute bed, or supporting Long term condition management.

This is a maternity cover for 12 months

The team comprises of qualified physiotherapists, and assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in community.

We have appointed a new Trainee Advanced Clinical Practitioner in community and there is an opportunity to develop an interest in frailty and long term conditions.

You will be working with patients with a large variety of conditions and disabilities, in their own homes and local community. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team.

Main duties of the job

The post holder is an autonomous practitioner, responsible for provision ofspecialised assessment and treatment of patientsreferred to the service. You will be responsible for managing a caseload of patients with a variety of complexconditions including long term condition management and acute hsopital discharges. You will see patients in own homes , working with an MDT in other services across East Lancashire, and other organisations. You will carry out specialist assessment and treatment including the prescription of essentialequipment as part of the therapeutic intervention .

You will play an important link with PCN and MDT teams in attending meetings as part of neighbour hood team working

The service operates Monday- Friday
