Job summary

The Cancer Services department have an exciting opportunity to join the team in this brand new role as a Cancer Quality Assurance Coordinator.

Under the direction of the Cancer Data & Performance Manager and Directorate Manager, the Cancer Quality Assurance Coordinator will be responsible for the cancer breach analysis process, ensuring there is a mechanism for the analysis and learning from breaches and near misses of the cancer waiting times treatment standards (both 62 and 31 day). This will involve the production of pathway timelines - mapping out a patients journey from clock start to clock stop, validating dates entered into the trusts cancer database, and categorising delays in line with trust policy.

The Cancer Quality Assurance Coordinator will ensure that analysis and learning from breaches and near misses is shared with the responsible directorates and where necessary is logged on an action plan for monitoring.

The post holder will, in collaboration with the Directorate and Clinical Leads, support the management of the trusts Cancer Action Plan to support improvement in cancer pathways and the trusts performance against cancer waiting times standards.

This post is fixed term for a period of 12 months and the post holder will be expected to quickly embed themselves into the team and make an immediate impact, therefore a good working knowledge of National Cancer Waiting Times Guidance and trust systems is essential.

Main duties of the job

Ensure the creation of breach analysis timelines for all breaches and near misses of the cancer waiting time treatment standard.

Validate data entered into SCR.

Work alongside the Cancer Data & Performance Manager to ensure that submission data is validated in time for the National CWT submission.

Submit incidents for any 62 day breaches that surpass 104 days or 31 day breaches that surpass 73 days.

Set up and attend Breach Analysis and Clinical Harm Review Meetings, ensuring breach reasons are identified and appropriate actions for improvement are assigned.

Identify actions/ areas for improvement/ lessons learnt.

Document actions/ areas for improvement/ lessons learnt in the trusts Cancer Action Plan and share with service improvement facilitators.

Maintain cancer action/ improvement plan.

Liaise with Multi-Disciplinary teams both within, and external to the trust to understand what led to patient pathways breaching.

Produce breach reason and 104 harm review reports on a monthly and quarterly basis and report these into the cancer triad.

Work with the Cancer Data and MDT Supervisors to identify gaps in CWT knowledge that have led to potential breaches and support with ongoing training and development of the teams.

Provide leadership and mentoring support to the Cancer Services team to promote a culture of continuous improvement and delivery of agreed objectives.
