Job summary

An exciting opportunity has become available for a Physiotherapist/ Occupational Therapist to join the Intensive Home Support Service (IHSS). The post holder will provide holistic assessments of both environmental and functional needs to maximise independence through compensatory techniques, low level equipment, urgent moving and handling equipment for people who are at risk of admission and safe in their usual place of residence. The therapist will also be required to carry out Musculoskeletal and respiratory examinations and use clinical judgement to formulate a treatment plan.

The IHSS team includes Physiotherapists, Nurses, Occupational Therapists, Social Workers, Paramedic, clinical and clerical support staff, and a Mental Health Practitioner. IHSS works closely with Hospital at Home which is supported by medics and nurse consultants.

IHSS provides proactive intervention for patients with acute and chronic long-term conditions with complex health and social care needs. All referrals follow a rapid 2-hour urgent community response model or 24-hour response to avoid unnecessary hospital admissions. The service works closely with other Community, primary and secondary care and Social Care Services.

The service operates over 24 hours, 7 days per week, 365 days a year including Bank Holidays. Band 5 Therapy shifts are within core hours 08:30 - 16:30 including weekdays and weekends on a rotational basis.

Main duties of the job

  • Comprehensive therapy assessment and management of patients on a clinical caseload including those patients with diverse or complex presentations/multi pathologies. To use advanced clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis.
  • To work collaboratively across Health and Social care, voluntary and independent sector to maintain partnership approach to care delivery.
  • To establish individualised management and treatment plans, using advanced clinical reasoning and utilising a wide range of evidence-based treatment skills and options to formulate interventions and programmes of care, which may include working with staff across organisational boundaries.
  • To contribute to the MDT's including Hospital at Home as needed.
  • Ensure that care and services are delivered in line with all relevant NSFs, CQC guidance, NICE guidelines, professional requirement, Trust policies and procedures, Standards for Better Health and current legislation.
  • You will be supported by Senior Therapists and other members of the MDT to complete training in clinical observations as part of all assessments utilising NEWS 2 (National Early Warning Score). It is also expected that generic risk assessments will be completed as part of assessments (eg, waterlow assessments FRAT and Abbey pain scale).
