Job summary

This is an exciting opportunity for an experienced and enthusiastic experienced nurse to join our team at ELHT. Previous breast care nursing experience is desirable but not essential.

The post holder will be responsible for providing high standards of care and holistic nursing to all breast cancer patients across East Lancashire.

They will need to be able to work autonomously as well as part of a team, they will be accountable for their own professional practice, they will give support, education and guidance to patients and cares throughout a patients cancer journey.

They will also support the band 7 CNSs and be able to liaise with other members of the MDT.

Main duties of the job

To assess, plan, implement and evaluate individualized programs of specialist evidence based nursing for the breast cancer patients and their families utilizing the agreed assessment tools for breast cancer patients in collaboration with other key professionals.

Support the medical staff in the out patient department, hospital wards and primary care settings with regard to breast cancer issues and agendas.

Responsible for the safe, personal and effective care of breast cancer patients by effective assessment of complex needs which may require specialist intervention.

Act as patient advocate to ensure the delivery of care is focused on the breast cancer patient and their family.

Support the close monitoring of cancer patients to meet waiting time targets.

Be actively involved with the MDT planning, implementation and outcomes.

Be aware of resources/referral criteria which enhance the patient/carer experiences during the cancer journey.

Working as a member of the Breast Care Team the post holder will provide specialist nursing care to patients with a diagnosis of breast cancer. They will be responsible for the co-ordination of the patients' treatment pathway, acting as a link between primary, secondary and tertiary care. They will act as a named key contact for patients, relatives and carers on behalf of the breast cancer MDT and provide support for patients and carers from the time of diagnosis onwards

Provide comprehensive education and advice to patients/ carers andcolleagues on issues relating to breast cancer management.

To maintain precise nursing documentation relating to patient care andensure that all clinical and legal documents completed are both accurate and legible and maintain confidentiality at all times.

Maintain own personal development to meet the requirements of the service.

Be able to work autonomously, prioritise and organise own specialistworkload.

Be able to work as part of a large multi-disciplinary team (MDT) and adapt to the role of mediator/ protagonist to improve service provision.

Help with the development and training needs of student and/ or othermembers of staff or general public.

Be aware of budgetary controls and ensure the economical use of resources, supplies and equipment.
