Job summary

There an exciting opportunity to join our team Digestive Disease Unit as a Band 6 Unit Sister/Charge Nurse.

Responsible for the assessment of patient care needs and the development, implementation and evaluation of programmes of patient care. Co-ordinate the Nursing team to ensure the effective delivery of care.

When delegated to do so; manage the Unit in the absence of the Senior Sister / Unit Managers. Give effective specialist nursing care to the patients on the Digestive Disease Unit. Demonstrate, supervise & lead learners in delivery of high quality Nursing & specialist care.

Show commitment to the 6 Cs of Nursing and demonstrate exemplary Nursing practise and national standards For NAPF.

Main duties of the job

The focus for the post holder will be in the following key areas:

  • Support the provision of a clean environment
  • Ensuring improvements in patient outcomes and the safety, effectiveness and experience of their care
  • Assist in the developing of nurse leadership, improvement, and management of services within the escalation ward area in order to ensure the highest standards of patient care and service are delivered in the most efficient and effective way.
  • Ensuring best practice in infection control
  • Treating patients with dignity and respect
  • Assist the ward manager in the provision of senior nurse leadership during both regular and unsocial hours
  • Assist the ward manager in ensuring all quality improvement work is implemented and sustained.
  • Ensure safe and effective discharge planning.
  • Supporting the delivery of the Nursing Strategy at all times.
  • Be a confident role model for the junior nursing team.

  1. Assessor of Nursing: -
  • 1.1 Establish an individuals functional capabilities in the context of an emergency presentation.
  • 1.2 Establish the details of the individual needing care
  • 1.3 Maintain accurate clinical observations of the patients condition invasively and/or non invasively
  • 1.4 Contribute to assessing individuals with suspected coronary heart disease
  • 1.5 Take a presenting history from an individual to inform assessment of an individual
  • 1.6 Assess individuals needs for psychological, emotional or social rehabilitation
  • 1.7 Conduct a pre-intervention assessment of an individual.
  • 1.8 Accurately complete all relevant documentations relating to patient assessment.

2.Planner of Nursing: -

  • 2.1 Prioritise and refer individuals for further assessment and care
  • 2.2 Support individuals through the process of dying
  • 2.3 Develop, implement and review care plans with individuals
  • 2.4 Co-ordinate the implementation of a treatment plan for individuals diagnosed with coronary heart diseases and other conditions.
  • 2.5 Assess presenting conditions and determine the appropriate intervention for the individual

3.Practitioner of Nursing: -

Givespecialistcare to patients and relatives:

  • Administer drugs and treatments, including mixing IV additives for infusion or administering IV bolus as prescribed with safety, accuracy and intelligencewithin the Trust Policy.
  • Assess patient care using the Trusts recognised assessment tools
  • Interpret the significance of clinical data
  • Able to effectively recognise peri-arrest / cardiac arrest situations using ABCDE approach and initiate appropriate therapy
  • Perform venepuncture and cannulation skills.
  • Adheres to National standards for Infection control & prevention. Including completion of current audit tools. Lead by example and challenge non-adherence to policy.
  • Prepare and manage unstable patients requiring escort out of trust for more specialised management.
  • After required training; be able to transfer patients to other hospitals & departments with Intra-aortic-balloon-Pump in-situ.
  • Access patients clinical results via ICE, PACS, Horizon, medcon with up-to-date password to ensure timely interventions according to clinical need.
  • Effectively add referrals to other MDT and Specialities
  • Communicate effectively with patients, their relatives and co-workers.
  • Liaise with medical, paramedical, staff and other members of the MDT in the delivery of care.
  • Take an active role in daily ward rounds, advocating for patient and able to provide reasoned arguments.
  • Manage patient flow in accordance with patient-need and 4 hour waiting targets for the benefit of patients in the wider Trust.
  • Demonstrate effective use of SBAR tool for patient handover in & out of the ward.

  • Promote and delivers health education activities within the ward.
  • Act as a learning resource for pre and post registration staff, and provide education about Digestive diseases, Liver disease , policies & procedures

7.Manager of Nursing: -

  • Organise staff & assign duties according to competence & experience, time manage workload and organise appropriate equipment required for the completion of work.

  • Report and receive reports of work activity.
