Main duties of the job
An exciting opportunity has arisen for an experienced Registered Nurse AFC Band 6) to join the Integrated Diabetes Service. The focus of this role will be to work closely with our multidisciplinary team (MDT) to support adult inpatients across ELHT, The post holder will support the management of inpatient's whilst working alongside a highly skilled team of Adult Consultants, Adult Diabetes Specialist Nurses, Specialist Podiatrist and Specialist Dietitians. ELHT have been successful in receiving NHS England bid monies to develop our inpatient service. The role is to prevent unnecessary admissions, support inpatient care and timely discharge, and delivery training to HCP across the health economy.
Our Integrated diabetes team is an early adopter of technology and uses examples of all current diabetes technology available to co-ordinate patient care, whether this is flash glucose monitors, insulin pumps and CGM. This role is to support 7 day working
