Job summary

To be the specialist clinician in the integrated Diabetes Service Foot Team which covers a wide range of care settings including hospital in-patient and outpatients setting, community hospitals over a 7 day period. To undertake therapeutic assessment of patients and work as part of a Multidisciplinary team providing high quality safe effective podiatric care top diabetes patients across East Lancashire Hospital Trust. Working with Diabetes Specialist Nurses, Orthotists and Health Care Professional and other support staff in the prevention of foot ulceration, management and potential reduction of lower limb amputation.

Main duties of the job

To work in a wide range of settings, to provide specialist advice via mobile phone and hot foot line. To access and screen diabetes patients using the diabetes foot assessment to undertake complex assessment and provide support to multi- professional teams such as GP's and practice nurses. To support the development of individualised programmes of care and wound management.

Job responsibilities

To provide specialist advice and develop individual treatment plan for patients, carers, Hospital Staff, District Nursing Staff, Treatment Room Nurses, Consultants, GPs, other healthcare professionals and Podiatry colleagues for diabetic patients with complex and highly complex diabetic foot conditions.

To assess, diagnose and provide a comprehensive range of treatments by means of various techniques and specialised equipment to manage various foot disorders for which appropriate training has been given and provide specialist programmes of care assessment and diagnosis to patients with complex pathologies (ischaemia, diabetes, neuropathy, and ulceration) following referrals direct from GP/Consultant, other healthcare professionals and Podiatry colleagues.

To undertake vascular assessment and be responsible for appropriate and timely referral of patients direct to Consultant Vascular Surgeons, or Vascular Specialist Nurses.

To offer educational support to clinical and medical personnel in the management of the Diabetic Foot and Lower Limb.

To provide training and mentorship to Podiatrists to ensure appropriate level of knowledge and skills in Diabetes.

To supervise / train / mentor Podiatry staff and students and other health care professionals.
