The Buckinghamshire Integrated respiratory service (BIRS) has an exciting opportunity within its service and looking for a dynamic allied healthcare professional or nurse with a keen interest in respiratory medicine.
A valid driving licence is essential for this role as the post holder will be required to travel to various BHT venues, community centres and patient homes within Buckinghamshire.
The post holder will provide specialist respiratory input supporting the service pathway within the acute, community setting and outpatient setting, using highly developed specialist practice underpinned by theoretical knowledge/respiratory qualifications.
Staff will be trained to be a specialist resource of information for health and social care staff, patients, families, carers, and external agencies who may require advice and support, and guidance in the management of respiratory conditions such as Chronic Obstructive Pulmonary Disease (COPD), asthma and other respiratory disease.
Main duties of the job
The post holder will work within the various service pathways as part of the Buckinghamshire Integrated Respiratory Team (BIRS). The role includes working as part of a multidisciplinary team to deliver education and treatment and management plans for patients with chronic obstructive pulmonary disease (COPD) and other respiratory conditions, visiting patients at home to reduce avoidable hospital admission, early supported early discharge pathways for patients with COPD and other respiratory conditions.
Provide advice and support to patients (and their carers where appropriate) including the provision of education and self-management support.
Support skills and knowledge development in community nursing and acute care.
Undertake patient assessments.
Provide specialist clinical knowledge and expertise in their related field.
Work in collaboration with the Team to look at new and innovative ways of providing a positive patient and staff experience. This will be demonstrated with the patient and staff experience feedback questionnaires.
Work closely and supportively with the Specialist Nurses and directly with the Department teams. This will allow facilitation to develop and improve nursing practice; improve staff morale and patient experience.
