Community Diabetes Specialist Nurse

Job summary

The Diabetes Specialist Nurse will act as the clinical expert and professional resource to the multi-professional team, patients and carers within their Speciality Service. This role is primarily community based.

Main duties of the job

The post holder will have a responsibility to help develop the specialist area of practice in partnership with other members of the speciality team working collaboratively to develop services and improve the quality of care delivered.

The post holder will carry responsibility for a defined case load and offer specialist clinical and service support and advice including setting standards of care, contributing to specific care guidelines, and supporting the care of patients across the health economy. The post holder will also have an active role in the education of patients, carers and healthcare professionals in their area of expertise.

The post holder will contribute to achieving the objectives of the clinical service group/speciality corporate objectives and work within the clinical governance framework. This will be achieved through the core dimensions of the role, clinical practice, leadership, acting as the patient advocate and through audit and education.

Essential

  • Registered Nurse
  • Non-medical prescribing
  • Evidence of further continued professional development
  • Reconised diabetes course
  • ENB998 or City & Gilds 730 or equivalent experience at level 3

Desirable

  • Care support planning (or willing to work towards)

Skills, Abilities and Knwledge

Essential

  • Computer skills
  • Able to work within differing client groups, particularly those with mental health and socially complex needs
  • Able to use adult education techniques to deliver education in groups or to individuals
  • Able to communicate clearly with patients, relatives, and a multidisciplinary team in English

Experience

Essential

  • Previous post registration NHS experience in a diabetes environment
  • Experience in using nonmedical prescribing in practice

Desirable

  • Audit and research experience
  • Experience in teaching patients new skills
  • Teaching and supporting junior staff and students
