Job summary
We are looking for a highly motivated pro-active individual who has excellent communication and interpersonal skills, shows initiative and excels in problem-solving, to be our operational lead for the system Discharge & Flow.
The role will coordinate and lead the multidisciplinary triaging of referrals for discharge pathways 1, 2 and 3 for residents of Buckinghamshire irrespective of where they are an inpatient: including out of area hospitals.
It will also support Pathway 0 for those patients requiring VCSE/Age UK support, including those who may be vulnerable or have safeguarding concerns e.g., homeless, disabled, end of life, escalating difficult issues and delays to the Head of system Discharge & Flow appropriately.
The postholder will oversee the operational management of our hub beds located across Buckinghamshire. These support people who no longer meet the Criteria to Reside in acute hospital beds but are unable to return immediately home from hospital. The postholder will also take a lead role in overseeing interim health discharge pathways for the people of Buckinghamshire, for example our non-weight bearing pathway.
Main duties of the job
The post holder will work closely with the Head of System Discharge & Flow, working across system boundaries in supporting discharges for patients who no longer meet the criteria to reside in hospital. The aim is to achieve timely discharge and flow of patients in a well-coordinated and integrated way, avoiding delays to returning home as well as reducing readmissions.
