Job summary
An exciting opportunity has been created for enthusiastic pharmacy technicians to be part of the Buckinghamshire Medicines Optimisation in Care Homes (MOCH) team working within the Integrated Elderly Care Community Care team in Buckinghamshire Community Trust. This has created the chance to improve care across health and social care for the patients of Buckinghamshire. It is an excellent career opportunity for a Pharmacy Technician to work collaboratively with care homes, community specialist teams, and Primary Care Network (PCN) clinicians, to make a real difference.
Main duties of the job
The post holder will work with a team of pharmacists in collaboration with care home staff, community specialist teams PCN clinicians and staff, and Social Care providers to improve care home and system-wide level medicines use as part of the Medicines Optimisation Agenda.
This includes medicines reconciliation and supporting pharmacists with post-hospital discharge medication reviews to help reduce hospital readmissions for care home residents. Training of care home staff is an important feature of the role.
In addition, the post holder will identify gaps and lead targeted actions in collaboration with the care home pharmacists to support the provision of high-quality, safe care for older people in care homes and the community.
The successful candidate must be a registered Pharmacy technician with the General Pharmaceutical Council. They will have current relevant and good knowledge and understanding of medicines optimisation, safety, legislation, and best practice in care homes as well as an understanding of interface issues with acute, community, and mental health organisations. They will have good interpersonal skills and demonstrate the ability to communicate with a range of colleagues, and health and social care professionals. Experience working in a care home setting is desirable but not a requirement.
