Job summary

This particular role provides an opportunity to work with the multi-disciplinary team to provide nurse led anticoagulation clinics across Buckinghamshire with experience of anticoagulation management.

Main duties of the job

  • Uses specialist knowledge and skills for assessing patients and their families and interpreting complex problems and provide the appropriate level of intervention.
  • Collect, collate, evaluate and report information, maintaining accurate patient records.
  • Involve patients and carers/relatives in the planning and delivery of care.
  • Complex decision-making, regarding anticoagulant dosage changes.
  • Monitor investigations results, initiating further assessment/review of treatment pathway as required.
  • Have expertise in managing patients with complications and queries related to anticoagulation.
  • Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to on-going care needs and discharge arrangements.
  • Establish and maintain effective communication with patients and carers/relatives, and professionals across health and social services.
  • Act at all times in accordance with the NMC Code of Conduct.
  • In conjunction with the Haematology Team initiate research projects.
  • Ensure safe and high standards of nursing care are maintained in accordance with hospital policies and procedures and national guidelines.
