Job summary

The Frailty Virtual Ward is our new innovative pathway which focuses on providing intensive, hospital- level care for acute conditions that would normally require an acute bed, in a patients place of residence for a short episode through multidisciplinary health care teams. This service model is continuously developing, to ensure the delivery of evidence based, community healthcare service 24hours a day, 7 days a week.

The Rapid Clinical Response team will provide support to patients in their place of residence following a sudden deterioration in their condition. The team will provide clinical treatment to patients and follow up on treatment on a daily basis ensuring effective treatment and recognising the need for increased support as required.

Main duties of the job

The post holder will work with a range of professionals in order to ensure care is appropriate and delivered in a timely manner. The post holder will be able to demonstrate their professional skills and competence in leadership, care planning / co-ordination, medicines management, and care and treatment delivery.

Key Areas:

  • To contribute to processes for proactively case finding patients who are very high intensity users of community and primary healthcare and / or are at high risk of unplanned admission to hospital
  • Assess the physical and psycho-social needs of the patients instigating therapeutic treatments and care delivery based on best available evidence in association with the Matron, Geriatrician and other team members.
  • To improve health outcomes using specialist nursing skills and knowledge
  • To manage a caseload of patients with acute needs
  • To plan, co-ordinate and evaluate care programme delivery using the single assessment process, managing patient referrals to ensure patients are treated by the most appropriate member of the team / service.
  • To develop protocols and procedures for clinical pathways
  • Participate in weekend rotas /7 day working rotas as required
