Job summary

An exciting opportunity has arisen for an innovative, hardworking and dynamic Band 5 Registered Nurse to work on ward 8 SMH.

If you are newly qualified or an experienced Staff Nurse with an interest in developing skills and knowledge in Acute Medicine for Older people , or would be interested in a new venture, we would love to hear from you.You will need to have excellent communication and inter-personal skills, and the ability to work both within a team and independently. You will be organised, have high standards of patient centred care and work efficiently under pressure.We offer a supportive and nurturing environment where professional development is encouraged and educational opportunities exist through in house and external training. Main duties of the job

To participate in the organisation and delivery of patient care services in such a manner as to contribute to the maintenance of the highest standards of care.Assists the Ward Manager to manage the nursing care of patients through the sequential process of care.Undertakes the nursing assessment of the patient including risk assessments, e.g.Tissue viability, falls and re - evaluates.Planning and delivering an individual programme of nursing care for each patient .

Supervising qualified and/or unqualified staff, to ensure the optimum application of skills necessary to provide the care planned for each patient.Recording nursing care given and evaluating its outcome in relation to achievement of objectives; modifying care plans accordingly.Discusses with and informs medical staff and other members of themultidisciplinary team, including external agencies when applicable, of patients' progress in relation to care given.Maintains a continuing dialogue with medical staff and the multidisciplinary team, including external agencies when applicable, on all matters relating to the welfare
