Cardiac Rehabilitation and Prevention Nurse

Job summary

The post holder will assist in the safe delivery of the cardiac prevention and rehabilitation service for the Buckinghamshire NHS Trust. As part of the duties they will carry out patients cardiac risk assessment, making appropriate and timely referrals to associated agencies and work with the patient to provide risk factor modification with the aim of reducing the risk of further coronary disease. They will support the Exercise Specialists in the safe monitoring and delivery of structured exercise programs and therefore may be required to work cross site.

Assist in the running of the cardiac rehabilitation service and promote continuous development in clinical practice and delivery of patient care, encouraging innovation and appropriate change in service delivery within the speciality of cardiac rehabilitation in response to patient need, audit and evaluation and national guidance.

Main duties of the job

Undertaking of face to face and telephone assessment for patients eligible for cardiac rehabilitation.

Assist with the delivery of Exercise classes - supporting the Exercise Specialists.

Liaising with clinical/medical teams, raising any medical concerns.

Attend Team meetings and SDU as appropriate.

Delivering health promotion, educating patients and family about modifiable risk factor to prevent further coronary events.

Assist with data collection and input of NACR data.Liaise with coronary care ward and day unit capturing data regarding elective and primary procedures.
