Job summary
An exciting opportunity has arisen for a Advanced Clinical practitioner to join our Specialist Frailty Services based at Colchester Hospital.
We are seeking a dynamic Advanced Clinical Practitioner (ACP) to join our multidisciplinary frailty team working predominately in the Emergency Department.
Bringing Frailty and older persons expertise to a busy and exciting department you u will play a key role in delivering elements of a Comprehensive Geriatric Assessments (CGA), with a focus on preserving function and independence, facilitating safe discharge, and supporting proactive patient care.
You will also support frailty services across other settings, including the Frailty Rapid Assessment Service (FRAS) and the Frailty Hospital @ Home service (Virtual Ward).
We would welcome applications from those within the last year of completing their ACP pathway.
Banding and Progression:
Band 7 if working towards a full MSc in Advanced Clinical Practice. (Due to the small amount of hours offered for this post study time would need to be taken initially in your own time but may be supported with increased funding at a later date)
Band 8A upon completion of a full ACP MSc, with clear progression to 8A once achieved, in line with Trust policy
Shift times are 08:00-18:00, weekend working as part of a rota will be expected.

Main duties of the job
Uses specialist knowledge and specific training to act autonomously in assessing, planning, and implementinga comprehensive management plan for undifferentiated presentations within their defined scope of practice.
Elicit a patient history appropriate to the clinical situation, which will demonstrate a holistic review of thepatient's physical and emotional wellbeing and needs.
Undertake physical examinations as indicated by the patient's condition; autonomously analysing complex clinical signs and investigation of results to consider differential diagnoses and diagnose conditions, initiating treatment as required.
Performs clinical procedures appropriate to the history and physical examination of the patient, including
further invasive testing and treatments requiring highly developed skills and precision, including (but not
limited to) catheterisation, cannulation, peripheral arterial blood gas sampling.
Requests investigations and interprets investigations such as blood, urine and other laboratory tests,
electrocardiographs (ECGs), ultrasound scans, X-Rays and computed tomography (CT) scans in accordance
with IR(ME)R regulations and as local policy allows.
